Print the application, or you may save the application for filling out later. Once you have completed the application with all the appropriate information requested, please fax to (907) 561-2063 or deliver to the ASMA office at 4107 Laurel Street in Anchorage.

ASMA Dues
ASMA Dues are not deductible as a charitable contribution for Federal income tax purposes but may be partially deductible as an ordinary and necessary business expense. 27% of ASMA dues is not deductible as an ordinary and necessary business expense, because this portion of dues is attributable to lobbying activities. Your ASMA membership includes subscriptions to the Alaska Medicine, Heartbeat and one free copy of the Medical Directory at no charge. ($30 of your member dues is allotted for Alaska Medicine, $15 for Heartbeat and $50 for the Medical Directory.)

50% of AMA Dues is not deductible as an ordinary and necessary business expense, because this portion of dues is attributable to lobbying activities. Your AMA membership includes subscriptions to the Journal of the American Medical Association (except fully retired members) and AMNews at no charge. ($20 of your member dues is allocated for JAMA and $15 for AMNews, $10 and $10 respectively for students.)

Alaska Medical Political Action Committee (ALPAC)
ALPAC contributions must be on a personal check. ALPAC contributions are not deductible as a charitable contribution.

Alaska Medical Alliance
The Alaska Medical Alliance is a statewide organization of physician’s spouses.

Physician Photos
Photographs are printed at no charge for ASMA members. Non-members who want their photo in the medical directory must pay a fee of $20 by the time the Directory goes to press (usually late November).

Information
Information is provided and verified by the individual physician. Neither the Alaska licensure, medical school graduation, nor specialty board certification is independently verified by the Alaska State Medical Association.

2008 Medical Directories
For a 2008 Directory Order Form, please call 562-0304 for one to be faxed to you. The handy, 276-page directories are expected to be available by the end of March. Reserve yours now! (Note: Some price changes have been made for the 2008 Directory.) 


HEARTBEAT - the "pulse" of ASMA
The 12-page Heartbeat newsletter is published bimonthly by ASMA. Subscriptions are $15 per year. Existing subscribers with address changes please call the ASMA office. Please submit items of interest or advertisements to Publications Director K.T. Kelly at kkelly.asma@alaska.net

Alaska Medicine magazine changes underway! The 40-page quarterly publication will become a larger annual magazine beginning in September of 2008. It will still feature the latest scientific studies in Alaska, but also contain more human interest articles about the state's physicians and the practice environment. Subscription and advertising details to be released soon. Items of interest can be submitted to Publications Director K.T. Kelly at kkelly.asma@alaska.net
